The undergraduate calendar outlines the rules and policies of the university and the faculty. It also includes course listings and descriptions.
Missed Coursework and Health Forms
If you have missed any coursework, please review our Missed Coursework Policy prior to submitting any of the forms listed below. For examples on missed coursework scenarios, review our Frequently Asked Questions page.
- McMaster Student Absence Form (MSAF)
- for coursework worth less than 25% of the final grade, resulting from medical or personal situations lasting up to three (3) calendar days.
- may only be submitted once per term.
- is available in the Mosaic Student Center in the drop-down menu under OTHER ACADEMICS.
- cannot be used for academic work that has already been completed/attempted; this includes the viewing and/or partial completion of an online assessment (quizzes, tests, etc.).
- MSAF – Administrative Report Form (Former Notification of Absence Form)
- for coursework worth more than 25% of the final grade, or if you have already used your MSAF this term.
- cannot be used for academic work that has already been completed/attempted; this includes the viewing and/or partial completion of an online assessment (quizzes, tests, etc.).
- McMaster Student Health Certificate
- in lieu of a doctors note, please have your family doctor or attending physician complete this certificate.
- Academic Accommodation for Religious, Indigenous, and Spiritual Observances Form (RISO)
- for coursework missed throughout the term due to Religious, Indigenous, and Spiritual Observances (see RISO Fact Sheet).
- Final Exam RISO requests must be submitted directly to Examinations.
- Midterm Test Conflict Form
- Application for Deferred Final Examination
- for missed Final Exams during the Final Examination Period.
- Application for Relief (Petition)
- an application form and petition letter are required to request relief from the Faculty’s Academic Regulations.
All absence forms must be submitted to the Student Experience Office as early as possible or within 24 hours of returning to classes.
Application for Relief (Petition) forms must be submitted to MacDrive. Please make sure that the file name includes your name and student ID number.
